The Companion assists the elders who are functionally, physically, and or socially impaired and require limited assistance and/or companionship. The Companion’s role is to assist elders with those activities of daily living that they are unable to perform without help, fostering at all times elder’s independence and freedom of choice. The care plans (duties) are defined by the program coordinator, elder and sponsor. This plan is coordinated/collaborated with the residential facility care supervisor.
The Certified Nursing Assistant provides basic nursing care to elders under the direct supervision of a licensed nurse or therapist.
The LPN Charge Nurse provides care to elders and supervises the day-to-day nursing activities provided by care managers or Certified Nursing Assistants. This position ensures medications are administered, documented, and provisioned properly.
The RN Charge Nurse provides care to elders and supervises the day-to-day nursing activities provided by Certified Nursing Assistants. This position ensures medications are administered, documented, and provisioned properly. They also provide nursing care and monitor the clinical status of elders on their unit.
The Shahbaz provides routine nursing care for elders in the Cottage, which houses a small group of elders (ten) sharing a central living area in a home-like setting. In addition to routine nursing care, the Shahbaz also coordinates the overall home keeping needs of the cottage unit (menus, groceries, clean-up, laundry, etc.).
